Western Wisconsin Cares (WWC), formerly La Crosse County CMO, a locally-based established agency, has provided services to members in Western Wisconsin since 2000!
WWC operates under a contract with the Wisconsin Department of Health Services to provide Long-Term Care Services under the Family Care Program to eligible residents of Buffalo, Clark, Jackson, La Crosse, Monroe, Pepin, Trempealeau and Vernon Counties.
WWC is committed to involving members in program development and service delivery. Members play an active role as part of the Interdisciplinary Team in meeting their identified needs and Long-Term Care Outcomes.
WWC strives to design a unique set of services for every individual to help keep members as independent as possible.
