Members and Families
Thank you for choosing Western Wisconsin Cares to provide for your long-term care needs or for those of a loved one. WWC strives to maximize member health and independence by making members active participants in the care planning process so we can provide the “right care at the right time.”
Upon enrollment, each WWC member becomes part of an interdisciplinary team that includes at minimum a registered nurse and a case manager. Other professionals and family members also may participate as needed or desired. The team then assesses the member’s needs and preferences, determines the “outcomes” the member wishes to achieve and creates an individual service plan to help achieve those outcomes.
The individual service plan specifies which services in the Family Care benefit package WWC will purchase for the member. The team also will work together with the member to coordinate other needed health care services.
