As a requirement of its contract with the state Department of Health Services to serve Medicaid-eligible residents with long-term care needs, Western Wisconsin Cares will subcontract with vendors to offer all services in the Family Care Benefit Package.
Provider agencies interested in contracting with Western Wisconsin Cares are required to submit an application.
