  • Weigh yourself & record weight on Chart
    • Same time each morning, after using bathroom
    • Same amount of clothing
    • Scale on flat surface
  • Eat and Drink less salt (sodium) Limit salt to <2000 mg sodium/ day (or as directed by your doctor)Heart Healthy Tips and Recipe ideas – American Heart Association
  • Take medications as directed
  • Be more active (check with doctor before starting any exercise program)
  • Limit Fluids to < 2 quarts each day or as directed by your doctor
  • Avoid alcohol

 Call Doctor or Clinic Nurse if:

  • You gain > 2-3 lbs overnight or > 5 lbs in 1 week or as directed by your doctor
  • You cough more often or have more trouble breathing
  • You need more pillows to sleep, or wake up at night short of breath
  • You feel lightheaded or dizzy
  • You notice more swelling in feet, ankles, or belly
  • You notice you have lost 10 lbs or more without trying to lose weight
  • You are getting tired faster
